1. You must fill out the Dan Promotion Order Form with each Promotion Packet you submit. 2. If the student has another nationality on their previous certificate, the student must submit proof of residency for at least six months for the promotion to be processed through federation/association 3. If the applicant has a prior certificate, a copy of the certificate must be attached to this form. The current Dan number must be the one given by Matsogi-do Board of Black Belt. 4. The Instructor of and applicant should ensure that no false statement will be found to the application, otherwise, the Dan will be nullified even after it’s issuance & the instructor will not be allowed to make further recommendations. 5. If anything is missing on this form it will be sent back to the federation /association for correction. 6. No parent or student should call Matsogi-do Dan office. Only instructors are to communicate directly with the Matsogi-do Dan Office.
